Do I need a Doctor's referral?

No referral necessary, you have direct access to physiotherapy services.


What should I wear?  

Please wear/bring comfortable, flexible clothing.  Depending on the area being assessed, short or t-shirt/tank top may be appropriate.  


Will my benefits cover physiotherapy?

Most benefit plans do cover physiotherapy, however each plan is different.  You may contact your benefit provider directly to determine your coverage.  


What if I was injured in a Motor Vehicle Accident?  

Everyone injured in a Motor Vehicle Accident (MVA) is entitled to coverage regardless of who was at fault.  

You will need to complete an AB-1 form and submit it to the insurance company.  

See a physiotherapist within 10 business days of the accident, they will diagnose your injury according to the Diagnostic Treatment Protocols The majority of MVA injuries qualify for 10-21 treatments within 90days of the accident, that can be billed directly to the auto insurance company.     

Occasionally, an injury falls outside protocol.  In that case, physiotherapy costs are billed to your extended health benefits first.  Any portion of treatment not covered by benefits will then be submitted to your car insurance company for reimbursement.

May I access physiotherapy for a motor vehicle ACCIDENT if it has been greater than 10 days from date of accident?

Yes, you can.  We will need to contact your insurance company to set up approval for treatment.  


Do you treat children and babies?

Absolutely!  We treat the full spectrum from infants to elderly.  Birth is traumatic even in the best cases.  Early years come with lots of falls and bumps to the body as we learn to crawl, walk, run and jump.  Clearing up any body restrictions in the early years will provide a more resilient body and calm nervous system.  We advocate just as strongly for preventative medicine as corrective care when there are acute symptoms.  


Do you treat work related injuries (WCB)?

Please note, we do not have a contract with WCB and thus, cannot treat work place related injuries.  


Will Alberta Heath Care fund my physiotherapy?  Post surgery/post fracture care?

Evolve Physical Therapy does not have a contract with Alberta Heath Services (AHS), and as such does not provide Alberta Health Care funded services.  You may be able to access AHS funded services for post surgery (within 56 days) and post fracture (within 2 weeks of cast removal) at another clinic.  If you choose, Evolve Physical Therapy can treat you for these privately or through extended benefits.  


What are my payment options?

Evolve Physical Therapy accepts MasterCard, Visa, E-Transfer (EMT) or cash. 


WHAT IS YOUR Cancellation Policy?

Cancellation Policy:  Cancellation of an appointment with 24 -4 hours notice are subject to a $50 fee. Appointment no-shows and cancellation with less than 4 hours notice are subject to a $90 fee.
